Job Description

The NSW Community Support Onboarding Manager is responsible for coordinating and overseeing all aspects of participant onboarding within the state. This includes facilitating initial participant meetings, managing the Quick Sign Up process, and developing tailored support plans based on individual needs and goals.

The role assesses participant needs, connects individuals and families with appropriate supports and services, and ensures the timely and effective commencement of services. The position plays a key role in ensuring participants feel supported, informed, and confident as they begin services with What Ability, while also contributing to broader community support outcomes.

Key Responsibilities

  • Provide direct, person-centred support to participants, assisting with daily living, personal care, skill development, and wellbeing
  • Attend participant bookings as a support worker to deliver services in homes, community, and social settings
  • Facilitate participant engagement in community activities, social events, and skill-building opportunities to promote independence 
  • Build strong, trusting relationships with participants and their families, providing guidance, advocacy, and emotional support
  • Conduct intake discussions with participants and their support networks to assess needs, goals, and preferences, and develop tailored support strategies
  • Develop, implement, and regularly review individualised support plans in line with participant goals and organisational frameworks
  • Monitor participant progress during bookings and support delivery, adjusting care and interventions as needed to ensure positive outcomes
  • Coordinate with support workers, internal teams, and external service providers to ensure participants receive consistent, high-quality, and person-centred care
  • Support participants in accessing community resources, programs, and services, fostering independence, choice, and social inclusion
  • Conduct onboarding-related activities
  • Facilitate staff training 

Skill & Experience

  • Experience working in the disability, NDIS, community services, or health sector
  • Proven experience coordinating participant or client onboarding, intake, or service commencement processes
  • Strong organisational skills with the ability to manage multiple onboarding timelines simultaneously
  • Excellent written and verbal communication skills, including engaging sensitively with
    participants, families, and internal teams
  • Experience maintaining accurate records and working within compliance or regulated service environments
  • Ability to handle confidential and sensitive information professionally
  • High digital literacy and experience using client management or CRM systems
  • Understanding of person-centred practice principles
  • Relevant qualification in Disability, Community Services, or a related field, or equivalent practical experience
