Job Description

At HI FIVE Community Services, we believe support should feel personal, not just professional. We’re a close-knit team that genuinely cares about the people we work with, and we’re committed to helping individuals living with disabilities live life on their own terms with dignity, choice, and independence.

For us, it’s not just about providing services; it’s about building real relationships. We take the time to understand each participant their goals, their routines, and what matters most to them. Whether it’s supporting everyday activities, building confidence, or encouraging community participation, we stand beside our participants every step of the way.

Our approach is simple: treat people with respect, show up with compassion, and do the little things that make a big difference. At HI FIVE, we aim to create a warm and supportive environment where participants feel valued and our team feels proud of the impact they make every day.

Because at the heart of what we do is one belief everyone deserves the opportunity to live a full and meaningful life.

Key Responsibilities

  • Provide assistance with personal care including showering, grooming, dressing, toileting, and mobility while maintaining dignity and individual preferences
  • Support participants to develop independence and life skills in line with their NDIS goals and daily routines
  • Assist with light domestic duties such as meal preparation, cleaning, laundry, and shopping to maintain a safe living environment
  • Facilitate community access by accompanying or transporting participants to social, recreational, or educational activities
  • Monitor participants’ wellbeing and report any changes or concerns to supervisors or relevant health professionals
  • Maintain accurate records of care, progress, and incidents in accordance with organisational and NDIS requirements
  • Assist with rehabilitation activities, therapeutic exercises, and basic treatments under the guidance of allied health professionals
  • Provide support to therapists during sessions and assist with medication administration where trained and authorised
  • Ensure compliance with NDIS Practice Standards, workplace health and safety, and mandatory reporting obligations

Skill & Experience

  • Certificate III or higher in Aged Care, Individual Support, or equivalent
  • Minimum 12 months’ relevant work experience required Australia/Overseas
  • Current First Aid and CPR certificate
  • National Police Check and Working with Children Check (or willingness to obtain)
  • Excellent communication and interpersonal skills
  • Ability to work independently and in a team
  • Empathy, patience, and a strong commitment to quality care

Desirable

  • Experience working with elderly clients or people with disabilities
  • Valid driver’s licence and access to a vehicle (for community care roles)
