Job Description

Hi Five Community Services is seeking a qualified Community Care Registered Nurse who speaks Hindi and Punjabi to join our team. The role involves delivering high-quality clinical support to a diverse client base, including clients who primarily communicate in Hindi and Punjabi, while ensuring safe, accurate, and culturally appropriate care delivery.

Key Responsibilities

  • Provide direct nursing care and clinical support to clients in home and community settings.
  • Conduct client assessments, care planning, and clinical documentation in line with regulatory standards.
  • Communicate clinical information clearly and accurately with clients and families, including those who speak Hindi and Punjabi as their primary language.
  • Liaise with allied health professionals, support coordinators, and family members.
  • Ensure compliance with relevant clinical governance, medication management, and infection control standards.
  • Maintain accurate and timely clinical records.
  • Train and upskill support workers in clinical care delivery for clients with high or complex care needs.

Skill & Experience

  • Current registration as a Registered Nurse with AHPRA.
  • Relevant nursing qualification (Bachelor of Nursing or equivalent).
  • Demonstrated experience in community, disability, or aged care nursing.
  • Current National Police Check and NDIS Worker Screening Check (or willingness to obtain).
  • Current First Aid and CPR certifications.
  • Valid driver’s license and own vehicle (if applicable).

Desirable

  • Experience working within NDIS or aged care service delivery.
  • Experience in culturally responsive care planning.
  • Ability to communicate in Hindi and Punjabi is highly desirable.

Salary & Employment Conditions

  • Salary: $90,000 per annum + superannuation
  • Employment type: Full-time, ongoing
  • Location: Western Melbourne Suburbs
