Job Description

Connections Home Care & Nursing is looking for experienced disability support workers who have experience supporting clients living with various disabilities.
Immediate start, training and development opportunities.
The successful candidate must:
· have a wide availability- Monday-Sunday
· be able to support clients throughout Melbourne
· have a current valid Victorian driver’s license and access to a car to transport clients (desirable)
· Non-drivers are also welcome to apply

Key Responsibilities

The support required for our clients include:

  • Assisting clients with daily living activities such as bathing, dressing, grooming, toileting, and meal preparation while promoting dignity and independence
  • Encouraging and assisting clients to participate in social, recreational, educational, and community activities to enhance inclusion and quality of life.
  • Helping clients develop and maintain skills such as cooking, household cleaning, budgeting, shopping, and using public transport to increase independence.
  • Observing and reporting changes in a client’s physical, emotional, or mental health, administering medication (where qualified and authorised), and following care plans.
  • Keeping detailed documentation of services provided, incidents, and client progress while ensuring compliance with workplace policies, safety procedures, and individual support plans

Skill & Experience

Preferred but not essential:

  • Certificate III or IV Individual Support
  • Certificate IV Disability
  • Certificate of IV Aged Care 
  • Nursing

  • Encourage and assist clients to participate in social, recreational, educational, and community activities to enhance inclusion and quality of life.
