Key Responsibilities

About you:

This is a rewarding role offering the opportunity to make a real difference in people’s lives. We are looking for:

  • Minimum 1 year experience in community services or social support
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Diploma of Community Services (or equivalent)
  • A compassionate, reliable, and proactive attitude
  • Ability to build positive relationships with clients and stakeholders
  • A commitment to promoting inclusion and independence

Skill & Experience

Tasks & responsibilities:

  • Advocate for clients’ rights, access, and inclusion within the community
  • Provide person-centred support tailored to individual needs
  • Develop and implement individualised support plans
  • Support clients with community engagement and participation
  • Promote independence, wellbeing, and life skills
  • Refer clients to appropriate specialist services and agencies
  • Maintain accurate documentation and progress notes
  • Work collaboratively with team members and external providers
