Job Description

The Community Worker provides person-centred community support services to older Australians living independently at home. The role involves assessing client needs, coordinating services, implementing support plans, advocating for clients, liaising with healthcare professionals and monitoring client wellbeing to promote independence, safety and quality of life.

Key Responsibilities

* Conduct comprehensive client needs assessments and identify physical, emotional, social and environmental support requirements.
* Develop, implement and regularly review individual care and support plans in consultation with clients, families and healthcare professionals.
* Coordinate delivery of home care, community care and allied health services to meet individual client goals.
* Liaise with general practitioners, hospitals, allied health professionals, community organisations and government agencies regarding client care.
* Refer clients to appropriate health, welfare, disability, housing and community services.
* Advocate for clients to ensure access to suitable supports and community resources.
* Monitor client wellbeing and reassess changing care requirements.
* Identify risks affecting client safety and escalate concerns in accordance with organisational policies.
* Maintain accurate case notes, assessments, incident reports and client records.
* Assist clients to increase independence, community participation and social engagement.
* Provide education to clients and families regarding available services, funding and community resources.
* Participate in multidisciplinary case conferences and contribute to care planning.
* Ensure services comply with the Aged Care Quality Standards, organisational policies and relevant legislation.
* Prepare reports and documentation required by management and government funding programs.

Skill & Experience

Good communication and interpersonal skills.
Able to follow care plans and maintain accurate client notes.
Ability to work effectively with clients, families and the care team.
