About the Role

On behalf of our client – a community-focused organization dedicated to empowering individuals with disabilities and mental health challenges – we are seeking a compassionate Youth Worker. The role centers on person-centered support, fostering independence, and connecting young people to community, education, and employment pathways.

Key Responsibilities

Key Duties and Responsibilities

Assess and Plan Support

  • Evaluate clients’ social, emotional, and physical needs to co-design individualized support and goal plans.
  • Develop and implement educational, training, and life-skills programs that promote self-advocacy and community inclusion.

Engage and Monitor Progress

  • Conduct strengths-based interviews to understand each young person’s aspirations and barriers.
  • Regularly review and report on client progress, adjusting strategies to ensure meaningful outcomes.

Refer and Liaise

  • Connect clients with specialist services – including housing, health, and employment agencies – to address complex needs.
  • Build partnerships with community groups, government bodies, and local businesses to enhance resource access and awareness.

Support Families and Vulnerable Groups

  • Provide family education and crisis support, particularly for young people with disabilities or those in out-of-home care.
  • Assist in adult service units and group housing, focusing on skill development and independent living.

Community Advocacy and Funding

  • Identify community gaps in welfare, housing, and recreation services, and advocate for inclusive resources.
  • Prepare funding submissions and reports for government and philanthropic bodies to sustain and expand programs.

Skill & Experience

Requirements

  • Relevant qualification in youth work, community services, or social work.
  • Minimum 1-year full-time experience in a related position.
  • Experience in disability or mental health settings, with strong case management and communication skills.
  • Relevant qualification to above field.
