About the Client

Our client delivers person-centred care to older Australians, supporting them to live independently with dignity. As a Residential Care Officer, you will coordinate holistic care plans, liaise with families and health professionals, and ensure residents’ wellbeing is prioritized. This role is pivotal in creating a supportive, home-like environment.

Key Responsibilities

Client Assessment & Care Planning

  • Conduct comprehensive assessments of residents’ physical, social, and emotional needs, and collaboratively develop individualized care and support programs.
  • Interview residents and their families to identify the nature and extent of difficulties, adjusting care plans accordingly.

Program Implementation & Monitoring

  • Implement educational, training, and lifestyle programs that promote engagement and wellbeing, including social activities and health education.
  • Monitor residents’ progress routinely, documenting changes and reporting outcomes to the clinical team and families.

Community Liaison & Referral

  • Identify community resources and services—such as health, welfare, housing, and allied health—and refer residents and families to additional support.
  • Liaise with community groups, government bodies, and private providers to raise awareness of available services and advocate for residents’ needs.

Reporting & Funding

  • Prepare detailed submissions for funding and resources, ensuring compliance with Aged Care standards.
  • Report to government agencies and other bodies on resident outcomes, service gaps, and program effectiveness.

Skill & Experience

  • Diploma or higher in Community Services, Aged Care, or related field.
  • Minimum of 1 year experience in aged care assessment, care planning, and case management.
  • Strong communication skills, with ability to engage families, agencies, and multidisciplinary teams.
  • Knowledge of My Aged Care, funding frameworks, and reporting requirements.
  • Current police check and driver’s license.
