About the Role

As a Residential Care Officer, you will deliver person-centered support, helping clients achieve their goals while ensuring their safety, wellbeing, and social connection. We are seeking a compassionate professional to join a team committed to excellence in home and community care.

Key Responsibilities

Client Assessment & Program Development 

  • Conduct thorough in-home assessments to identify clients’ physical, emotional, and social needs, then design individualized support, training, and educational programs.
  • Interview clients and their families to evaluate the nature and extent of difficulties and collaboratively develop goal-oriented care plans.

Monitoring, Referral & Reporting 

  • Regularly monitor client progress against care plans, documenting outcomes, and adjusting interventions as required to promote independence.
  • Refer clients to appropriate health, welfare, housing, or employment agencies to access additional services and resources.

Community Liaison & Support 

  • Liaise with community groups, government bodies, and private providers to raise awareness of available resources and address local service gaps.
  • Support families by providing education, respite, and guidance for caring for children or adults with disabilities in home and group settings.

Supervision & Crisis Assistance 

  • Assist young people and vulnerable adults to resolve social, emotional, or financial challenges through practical advice and advocacy.
  • Prepare funding submissions and detailed reports for government and funding bodies to secure resources for client programs.

Skill & Experience

  • 1 year of full-time work experience within community services, aged care, or disability.
  • Relevant qualifications are required.
  • Strong communication, documentation, and interpersonal skills.
  • Valid driver’s license and willingness to travel to client homes.
